Hospital Discharge Teams
Packages can be set up within 24–48 hours of discharge. We have experience with complex discharges and can adapt quickly to changing needs.
GPs & Primary Care
When a patient needs support at home following illness or deterioration, we can be the bridge between clinical care and daily living support.
Occupational Therapists
We implement OT recommendations in the home, supporting safe transfers, equipment use and following care plans to the letter.
Dementia Advisers
Our carers receive dementia awareness training and work sensitively with clients and families through every stage of the condition.
Third Sector & Voluntary Orgs
